FTM Surgery

Female to Male Surgery Overview

FTM Surgery includes a spectrum of surgical procedures that transform female anatomy into anatomy that is more appropriately masculine.

FTM Top Surgery

The most frequently accessed FTM surgery is bilateral mastectomy (removal of the breasts) and male chest contouring, popularly referred to as FTM Top Surgery.

There are several Top Surgery methods. Those with a very small chest can have a Keyhole procedure, which uses liposuction and a small crescent incision at the bottom of the areola. A Peri-Areolar approach is often used for those with medium sized chests. Large chested men typically have Double Incision Top Surgery, which includes free nipple grafts.

Regardless of the technique used, the outcome goal is the same: an expertly flattened and contoured masculine chest.

Hysterectomy

While many transgender men opt for no surgery or just Top Surgery, many others also pursue bottom surgery, which includes Hysterectomy. This procedure removes the uterus, and may also include removal of the cervix, ovaries and Fallopian Tubes.

There are multiple surgical techniques for Hysterectomy: Laparoscopic, Abdominal and Vaginal Hysterectomy.

Hysterectomy results in reproductive sterilization and is a required surgery if further procedures—Metoidioplasty or Phalloplasty—will be performed.

Metoidioplasty

Metoidioplasty is a FTM Surgery procedure that takes advantage of testosterone’s enlarging affect on the clitoris and results in a small phallus that retains native erectile function. Commonly referred to as a “meta,” the procedure can also include urethral lengthening, enabling the ability to stand-to-pee, as well as implantation of testicular prothesis.

Metoidioplasty is less invasive and less expensive than Phalloplasty. The trade-off is penis size, which averages 1.5″ – 2″ with metoidioplasty, but the procedure remains a good choice for those who want to masculinize their genitals without going through Phalloplasty.

FTM Phalloplasty

FTM Phalloplasty is a surgery procedure that creates an adult-sized penis using a pedicled or free flap. Common donor sites include the forearm, back, thigh, and abdomen.

Phalloplasty surgery may also include urethroplasty (lengthening of the urethra to the tip of the phallus), as well as implantation of testicular and erectile prothesis.

Dr. Rumer uses a pedicled abdominal graft that is tubed to create the phallus. Patients who would like to have the ability to stand to urinate can opt for urethroplasty. Learn more »
